Conditions:
- If you have existing License Plates with our previous logo you can contact the Dept of Transport direct and request a remake of your plate with our new artwork.
- You can choose up to 4 digits for your very own plate.
- Include 3 preferences when placing your order and payment.
- A Dept of Transport (DoT) application form will be forwarded with confirmation of your selection.
- The DoT application form will need to be signed and completed and emailed back to the club.
- Your application form with payment will be forwarded to the DoT for manufacturing.
- DoT will advise by mail when your plates will be available for collection at the licensing centre.
- Perth Glory will refund in full if your selection is not available.
